Techniques for the management of meniscal pathology are evolving. In the past decade, the volume of literature describing indications, techniques, outcomes, and complications associated with meniscal allograft transplantation has increased exponentially. Approaches and techniques vary, and fixation strategies include soft-tissue only fixation, bone tunnel fixation, and bridge-in-slot fixation. Regardless of the technique selected, graft preparation and insertion are critical components of the procedure. This video describes the indications for, workup for, techniques for, and outcomes of patients undergoing arthroscopic-assisted bridge-in-slot meniscal allograft transplantation. Surgical pearls, including patient positioning, surgical approaches, and portal placement, are reviewed in detail.
