Removal of Well-fixed Revision Components
This operative technique video follows a 65-year-old female with multiple prior revision total hip arthroplasties, chronic dislocation, instability, and a history of prosthetic joint infection. The surgeon demonstrates a systematic approach to removing well-fixed revision components: careful exposure of the femoral stem, progressive disruption of the bone–implant interface using burrs and long-tipped instruments, conversion to an extended trochanteric osteotomy when direct extraction fails, and controlled removal of the acetabular shell and augment with minimal bone loss. The osteotomy fragment is reduced and secured with cables, and the patient is managed with protected weight bearing until osteotomy healing.
