Anatomic posterolateral corner reconstruction has been biomechanically validated to restore varus and external rotational stability to the knee. Clinical outcome studies have demonstrated that the anatomic reconstruction technique results in improved patient outcomes. The anatomic technique involves the use of two grafts to re-create the three static stabilizers of the lateral side of the knee: the fibular collateral ligament, the popliteal tendon, and the popliteofibular ligament. Two-graft anatomic posterolateral corner reconstruction also allows for passive range of motion exercises early in the postoperative rehabilitation program, which decreases the risk of postoperative arthrofibrosis without compromising clinical results. This video demonstrates anatomic posterolateral corner reconstruction in a stepwise manner. In addition, the video presents the indications for surgery and clinical outcomes.
