Iliotibial Band-Sparing Total Hip Arthroplasty via a Superior Portal-Assisted Approach
This video shows iliotibial band-sparing total hip arthroplasty via a superior portal-assisted approach. This approach is minimally invasive and spares the iliotibial band; however, it can be easily converted to an extensile approach during the initial learning curve period, if necessary. The hip joint is entered via a superior capsular approach. The tip of the greater trochanter is used as a reference for preparation of the femur. Preparation of the acetabulum is achieved via a percutaneous portal, which allows for visualization of the entire rim of the acetabulum during reaming and placement of the cup. Preoperative and intraoperative digital radiographic measurements are essential to achieve target component positioning, limb length, and offset. The piriformis tendon, conjoined tendon, and capsule typically can be repaired at the end of the procedure. The educational objectives of this video are to demonstrate iliotibial band-sparing total hip arthroplasty via a superior portal-assisted approach and the manner in which digital radiography is used to achieve target component positioning, limb length, and offset.
