Uniplanar Subtrochanteric Femoral Shortening Osteotomy for Leg-Length Discrepancy
This video demonstrates a technique for uniplanar subtrochanteric femoral shortening osteotomy for the treatment of a patient with a complex leg-length discrepancy. Given the advent of minimally invasive limb-lengthening procedures, a femoral shortening osteotomy is indicated less often. However, a femoral shortening osteotomy of the contralateral limb is a useful adjunctive corrective procedure in patients with severe anisomelia in whom a maximally lengthened limb has not resulted in adequate correction. This video discusses the case presentation of a patient with a severe leg-length discrepancy (10.4 cm) as a result of multifocal osteomyelitis affecting the distal femoral physis. After multiple lengthening procedures, a uniplanar femoral shortening osteotomy with intramedullary nail insertion was performed to correct the remaining 2.8-cm leg-length discrepancy. The video reviews the indications, alternative treatment options, preoperative planning, surgical technique, and postoperative outcomes.
