Arthritis of the lesser tarsometatarsal joints (TMT) is a pathology frequently encountered during foot and ankle consultation and may lead to disability and chronic pain. It may be present alone or in combination with other foot deformities. After exhausting nonsurgical management, the standard of care is surgical management consisting of arthrodesis of the affected joints. Many surgical techniques have been described for the management of first TMT arthrodesis; however, few exist for the management of the lesser TMT joints. The authors of this video propose a technique that involves the use of the same trephine to harvest the autograft from the calcaneus and to prepare the fusion site at the TMT joint. After the graft is harvested, it can be packed into the site of arthrodesis. A staple is used to maintain fixation, taking advantage of its compressive properties.
