This video describes the case presentation of a 13-year-old girl who sustained a both-bone forearm fracture with resultant entrapment of the median nerve within the radius fracture site. The patient had persistent symptoms of median nerve compression after fracture union. Diagnosis was confirmed via MRI preoperatively. Surgical management involved median nerve exploration, neurolysis, and nerve sheath tunnel placement. At 6 months postoperatively, the patient reported improvements in pain and paresthesias but had residual numbness in the index finger and thumb. Median nerve entrapment after both-bone forearm fracture is a rare complication that should be considered in patients with persistent neurologic symptoms postinjury. MRI may be useful for early diagnosis and surgical planning.
