Wide Resection of Chest Wall Tumor: Subtotal Claviculectomy and Partial Resection of the Manubrium and First Rib
This video provides a comprehensive description of the surgical technique for wide resection of a chest wall tumor. The patient discussed in this video is a 73-year-old man. In 2017, the patient was diagnosed with basal cell carcinoma of the anterior chest about the sternum and left sternoclavicular joint. He was treated definitively via radiation therapy. In 2022, surveillance MRI suggested local recurrence, and needle biopsy confirmed recurrence of the carcinoma with basosquamous differentiation. CT scans revealed a lucent lesion in the left manubrium and a pathologic clavicle fracture. Wide en bloc surgical excision of these areas and partial first rib resection, including the surrounding soft tissues, were recommended. The defect was left flail, with coverage achieved via a free muscle flap and a skin graft.
